We’ll write to you to let you know the outcome of your claim or medical review.

If you want a copy of your report, call our Disability, Sickness and Carers line. We’ll let you know if you need to make a formal Freedom of Information request.

Sonic HealthPlus aren’t able to provide a copy of your Disability Medical Assessment (DMA) report to you, this can only be provided by us.
