You need to provide us with certified evidence of your ongoing membership with your professional association. This makes sure we don’t deactivate your Healthcare Provider Identifier-Individual (HPI-I).

We won’t deactivate your HPI-I if you:
- provide certified evidence of your ongoing membership with your professional association
- have added other allied health professions to your HPI-I that remain current.
You can use the following process to provide evidence of your ongoing membership with your professional association or to reactivate your HPI-I.
You need a Provider Digital Access (PRODA) account to access Health Professional Online Services (HPOS). If you don’t have one, register for PRODA online. Then follow the steps to set up your HPOS access:
- Log in to PRODA to use HPOS.
- Select My programs.
- Select Healthcare Identifiers and My Health Record.
- Select Healthcare Identifiers - Register Allied Health Professional.
- Select Provide evidence of ongoing professional association membership to my existing HPI-I and then Apply now.
- Select your professional association and then Next. If you can’t find it in the list, select the My professional association is not in the list.
- Enter the additional information we may need to complete your request.
- Upload your certified evidence of professional association membership and select Submit.
- Make sure your details are correct, you can amend any errors by selecting Previous.
- If you agree with the Privacy Note, select I agree and then Confirm.
Once we’ve processed your request, we’ll send the outcome to your HPOS mailbox.
