Steps to claim a reimbursement
1. Claim from your private health insurance if applicable
If you have private health insurance, you need to claim any refund you’re eligible for from them. You need to do this before you claim a reimbursement from us. Read more about claiming when you have private health insurance.
If you don’t have private health insurance you can claim straight away from us.
2. Get ready to claim
Download and complete the External breast prostheses reimbursement claim form.
If you can’t print the form, you can either:
- call the Medicare program and ask them to send you the form
- go to a service centre and pick up a form.
To support your claim you’ll need to give us a copy of your receipt with a description of what you bought.
3. Make your claim
To make your claim, you need to submit your:
- claim form
- purchase receipt with a description of what you bought, the date and supplier information.
You can scan and email everything to the address on the claim form.
You can also submit your claim by post or at a service centre. All the details are on the form. If you submit your claim at a service centre, we’ll copy your receipt and give it back to you.
4. Wait for the results
We process most claims within 10 business days after we get the claim.
We’ll send you a payment statement if your claim is successful.
If we reject your claim, we’ll send you a letter explaining why. You can appeal our decision. Read more about reviews and appeals.
