Submit a manual Medicare and DVA claim

Submit a manual claim for payment for patient claims if you’re unable to submit a Medicare or Department of Veterans’ Affairs (DVA) claim online.

Submit a manual Medicare claim

For patient claims ask your patient to download and complete the Medicare Claim form (MS014). Your patient can use this form to claim benefits by mail or in person at a service centre.

Private health insurers or approved billing agents can use the Simplified Billing manual claims batch header form (HW091) to submit a manual Simplified Billing claim.

Submit a manual DVA claim

You can submit a manual DVA claim by mail.

It’s important you send all the required documents with your claim otherwise there’ll be delays in payment.
