Remove someone from a Medicare card

You can update your card when your circumstances change.

If you need to make changes to the people listed on your Medicare Card and you are worried about your safety, we may be able to support you.

Speak with one of our social workers if you are in, have left, or are preparing to leave a family and domestic violence situation. They can help you with supports, like getting your own Medicare Card.

You can visit one of our service centres or call us on 132 850 to talk with a social worker.

Remove someone from your card

If your family circumstances change, you can get your own card. You may want to do this if you break up or separate.

You can’t remove someone aged 15 or older from your current card without their consent.

Leave a card

You can leave a card if you have 2 Medicare card numbers and you only use one of them. For example, you have your own card and you’re still on your parents’ card.

If you’re the contact person on the card you’re leaving, we’ll assign a new contact person.

Remove children from their current card

You can’t remove a child 15 or older from your card. However they can get their own card.

You can remove a child under 15 from their current card to another Medicare card. Both parents or guardians must complete the Application to copy or transfer from one Medicare card to another form.

If either parent or guardian cannot complete the consent declaration on the form, they must give supporting documents. For example:

  • Apprehended Violence Order
  • Family Court Order showing parental responsibility.

What you need to do:

  • Complete an Application to copy or transfer from one Medicare card to another form.
  • List all the children you want to copy or transfer and if you want to remove them, you’ll need to select ‘transfer’.
  • Anyone 15 or older must complete the declaration on the form.
  • The applicant listed as Person 1 must give identification. Types of identification you can give are on the form.
  • Give supporting documents if both parents cannot complete the consent declarations on the form. Types of supporting documents you can give are on the form.
  • Send your form and supporting documents to the email and postal address provided on the form.
