This form can also be used to claim gap benefits from your private health fund for in-hospital and ancillary services.
If your myGov account is linked to Medicare you can submit your claim online.
To do this:
- Complete a Medicare Two-way claim form.
- Sign in to myGov.
- Attach an image of your original accounts and receipts if paid.
- Attach an image of the completed Medicare Two-way form.
- Submit your claim.
