How to upload a medical certificate

Instructions on how to upload and submit a medical certificate using your Centrelink online account or the Express Plus Centrelink mobile app.

Upload medical certificates using your Centrelink online account

You can upload either a Centrelink or non-Centrelink medical certificate.

We accept jpg, png or pdf file formats. Each file must be less than 10 MB in size.

Centrelink Medical Certificate form (SU415)

If your Centrelink online account is linked to myGov, sign in now and follow these steps to upload a Centrelink Medical Certificate:

  1. Make sure your doctor has signed the certificate before you upload it.
  2. Scan or save the completed Centrelink Medical Certificate on your mobile device or computer.
  3. Sign in to myGov.
  4. Select Upload documents from your homepage, then select Begin.
  5. Choose Select a document type.
  6. Select Centrelink Form.
  7. Enter the form code as SU415, then select Continue.
  8. If your upload consists of more than one file, you can drag and drop your files into the Drop files here box or choose Select your file.
  9. If you choose Select your file, find the file or files you want to upload from your device.
  10. Select Next if you don’t have any more files to upload.
  11. Review the information to make sure it’s correct. If you need to delete a document, select the bin icon.
  12. When your document is ready, select Next.
  13. Read and accept the declaration, then select Submit documents.

Sign in to myGov

Non-Centrelink medical certificate

If your Centrelink online account is linked to myGov, sign in now and follow these steps to upload a medical certificate:

  1. Make sure your doctor has signed the certificate they’ve provided you before you upload it.
  2. Scan or save the medical certificate you want to upload on your mobile device or computer.
  3. Sign in to myGov.
  4. Select Upload documents from your homepage, then select Begin.
  5. Choose Select a document type.
  6. Select Other documents.
  7. Select Medical certificate from the dropdown menu, then select Continue.
  8. If your certificate includes more than one file, you can drag and drop your files into the Drop files here box or choose Select your file.
  9. If you choose Select your file, find the file or files you want to upload from your device.
  10. Select Next if you don’t have any more files to upload.
  11. Review the information to make sure it’s correct. If you need to delete a document, select the bin icon.
  12. When your document is ready, select Next.
  13. Read and accept the declaration, then select Submit documents.

Upload medical certificates using the Express Plus Centrelink mobile app

Select Upload documents from the Frequently used section on your home screen.

To upload a medical certificate, select Upload something else.

If you don’t have any required tasks, select Upload document.

You can then follow the steps to upload either a Centrelink or non-Centrelink medical certificate.

Centrelink Medical Certificate form (SU415)

To upload this type of medical certificate, follow these steps.

  1. Make sure your doctor has signed the certificate before you upload it.
  2. You need to select Centrelink form.
  3. Use your device to Scan barcode to capture the barcode at the bottom of the Centrelink form.
  4. When you select Scan barcode, the camera on your device will open.
  5. Move the borders around the barcode, then your device will scan the barcode automatically.
  6. Select Next to upload your certificate.
  7. Select one of these options.
    • If you’re using an Apple device:
      • Take a photo to capture an image of the document.
      • Upload a document to choose a document you’ve already saved in your device or in the cloud.
      • Add a photo from library to choose an existing photo from your device's photo album or gallery.
    • If you’re using an Android device:
      • Upload a photo to capture an image of the document.
      • Upload a document to choose a document you’ve already saved in your device or in the cloud.
  8. When your document is ready, select Submit.

Non-Centrelink medical certificate

To upload this type of medical certificate, follow these steps.

  1. Make sure your doctor has signed the certificate they’ve provided you before you upload it.
  2. Select Other document.
  3. Select Medical certificate from the dropdown menu, then select Next.
  4. Select one of these options.
    • If you’re using an Apple device:
      • Take a photo to capture an image of the document.
      • Upload a document to choose a document you’ve already saved in your device or in the cloud.
      • Add a photo from library to choose an existing photo from your device's photo album or gallery.
    • If you’re using an Android device:
      • Upload a photo to capture an image of the document.
      • Upload a document to choose a document you’ve already saved in your device or in the cloud.
  5. When your document is ready, select Submit.
  6. Read the declaration, then select I accept.
