When to use this form
Only use this form if your organisation such as an aged care home, hospital or hostel, is approved for group payment arrangements.
If you do this, the organisation will deduct their fees from your payment and give you the balance or put into trust for you.
You or the organisation can use this form to remove your payment from the group payment arrangement. You will need to contact us and we’ll update your bank account details to make sure your payment is going where you want it to.
When not to use this form
Don’t use this form if your organisation isn’t approved for group payment arrangements.
