Chronic disease care plans

If your patient has a chronic medical condition, they may be eligible for services under any or all of the following:

  • General Practitioner Management Plan (GPMP).
  • Team Care Arrangements (TCAs).
  • Mental Health Case Conferences (MHCC).

Chronic medical conditions are those that have been, or are likely to be, present for at least 6 months.

Continence Aids Payment Scheme (CAPS)

CAPS helps eligible people with permanent and severe incontinence with some of the costs of continence-related products.
