You can use this form to request PBS prescription pads if you are an eligible prescriber including:
- Medical practitioners
- Nurse practitioners
- Midwives
- Optometrists
- Dentists.
This order form must be typed. We will return handwritten, incomplete or poor quality forms to the prescriber.
