You must give us both of the following. Proof:
- of your medical equipment needs from a medical practitioner, this can be a letter from your doctor
- you or the person you care for pays the energy running costs, this can be an electricity or gas bill.
We may also ask you to give us one of the following. Proof that:
- you qualify for help from a state or territory government scheme
- the Department of Veterans’ Affairs, through the Rehabilitation Appliances Program, supplied the medical equipment.
