How to complete a medical certificate for your patient if they can’t meet the requirements for their Centrelink payment due to sickness or injury.

There are different ways you can complete a Centrelink Medical Certificate.

You can either:

  • submit a Centrelink Medical Certificate (SU683) online using Health Professional Online Services (HPOS)
  • complete a Centrelink Medical Certificate (SU415) form and sign it.

We prefer a Centrelink medical certificate form (SU683) as this helps us assess your patient’s request faster. Only general practitioners can complete this task.

You need a Provider Digital Access (PRODA) account to submit Centrelink forms through HPOS. If you don’t have one, register for a PRODA account. Then follow the steps to set up your HPOS access.

Once you’ve completed the Centrelink Medical Certificate (SU683) form through HPOS, it’s submitted on behalf of your patient.

Steps to complete a Centrelink Medicare Certificate (SU683):

  1. Select My programs from the HPOS home screen and then Centrelink forms.
  2. Select Medical Certificate (SU683).
  3. Read the Medical Certificate instructions, complete the declaration and then select continue.
  4. Enter all relevant details to complete the patient’s Medical Certificate and select Save and continue after each section. All mandatory fields must be completed to continue. Mandatory fields are indicated with a red asterisk.
  5. Check the information you’ve entered in the Review and submit screen. You can use the Update option for each heading of the form to go back and correct any details.
  6. If all the details are correct, select Submit.

Download and complete the Centrelink Medical Certificate (SU415) form by hand.

Then give the completed form and any supporting documents to your patient so they can either:

  • use their Centrelink online account to upload them
  • lodge them at service centre.

You need to sign the form before giving it to the patient.

Authorised medical doctors can complete their own medical certificate form. We’ll accept this as long as it has the required details.

The certificate needs to include all of the following:

  • the date you completed the medical certificate
  • what the illness, injury or disability is
  • how long it will take for them to recover
  • the period they’ll be unable to work, participate or study
  • your signature on the form.

It also needs to tell us how many hours the patient can work, study or participate each week or if they’re unable to do these for at least 8 hours per week.
